Utilize the Community Health Inclusion Index (CHII) Organizational Assessment to examine how your policies, spaces, and programs provide equal access to all, including people with disabilities. Developed by the National Center for Health, Physical Activity, and Disability (NCHPAD), this survey is part of a larger index designed to help communities and organizations assess and improve their level of inclusion.
